Forms Related to Classes
- Appeal to Repeat Form Repeat a class you received a C or better in, or repeat a class for the third time
- Independent Study Contract (PDF) Approve your independent study course plan
- Drop Classes Form Withdraws you from your last class in the semester. Log into office.com using your UWO credentials. You can drop some classes on your own in TitanWeb if you’re keeping at least one course.
- Step Away for a Semester or Permanently Withdraw Form Withdraws you from classes this semester, in a future semester, or permanently. Log into office.com using your UWO credentials before completing the form.
- Social Work Field Work Approval Form (PDF)
- Special Education Field Work Approval Form (PDF)
Forms Related to Changing Your Program or Program Emphasis
- Change your emphasis within your degree (PDF) For example, change emphasis from Biology to Microbiology
- Change your Academic Program (PDF) For example, change from Cybersecurity, MS to Social Work, MSW
Forms related to Completing Your Certificate or Degree
- Time to Degree Completion Extension Form (PDF) Request approval for more time to finish your degree or certificate. You’ll need a form if the time taken from the first course to the last course exceeds:
- Four (4) years for certificates and Psychology masters; Five (5) years for Nursing masters or doctorate degrees; Seven (7) years for all other masters or doctorate degrees
- Instructions to Apply for Graduation (PDF) for each certificate or degree you earn
- Admission to Candidacy instructions and form (PDF) for students earning a masters or doctoral degree
- Curriculum Modification Approval form (PDF) required along with the Admission to Candidacy if any substitutions or exceptions to the approved curriculum are reflected on the Admission to Candidacy
- Verification of Program Study information and form (PDF) for students earning a certificate or GAP
- Curriculum Modification Approval form (PDF) required along with the Admission to Candidacy if any substitutions or exceptions to the approved curriculum are reflected on the Admission to Candidacy
- Change classes in your previously approved Admission to Candidacy or Verification of Program Study plans (PDF)

Policies
Graduate Policies
The Graduate School official policies are listed in the biennial Graduate bulletin. Bulletins are published every even year, and are effective Fall term. Use the bulletin in effective for your admit term.
For example:
- Admitted between Fall 2024 and Summer 2026, use 2024-26 bulletin
- Admitted between Fall 2022 and Summer 2024, use 2022-24 bulletin
Appeals Process
Students interested in appealing an administrative decision or policy should follow these appeals procedure.
Semester/Term Withdraw
If you want to withdraw from all courses in a semester:
- Log into office.com using your UWO credentials
- Submit the appropriate withdrawal form on this page
Be aware of the following about a semester/term withdrawal.
- Talk with the program coordinator/advisor about the withdrawal. The coordinator/advisor needs to approve the request
- A withdrawal from all courses does not mean an automatic refund of tuition and fees. Please review the refund dates and restrictions on the Student Accounts/Financial Services website to know whether any refund is possible.

Course Grade Designations
Grade point averages are determined by dividing the number of grade points earned in courses by the number of credits attempted in those courses. Point values are related to the various letter grades as follows:
A: 4.00; A-: 3.67; B+: 3.33; B: 3.00; B-: 2.67; C+: 2.33; C: 2.00: F: 0.00
The grades of C-, D+, D and D- are not used in graduate courses. There are several other grade designations assigned to courses in special circumstances. See the course syllabus or course instructor for details.
